For many people, it is very difficult to be anxious; while under the influence of MDMA, people are almost not afraid. 99% of people under the influence of MDMA feel safe to euphoric, with everything in between. However, there are exceptions. It does not happen often, but some feel anger or fear during an MDMA session. MDMA can therefore often significantly reduce anxiety, but not completely eliminate it for everyone.
The most accurate answer, therefore, is that MDMA significantly reduces anxiety in many people, but does not eliminate it completely. The likelihood of anxiety is usually lower than without MDMA, but not zero. That is why a good setting, trust in the facilitator, preparation, and a quiet environment remain important.
For people with a lot of underlying tension, trauma, or mistrust, it is especially wise to have a plan in place beforehand for moments of unrest.
