MDMA + Tramadol results in:

  1. Neurochemical interactions: MDMA increases the concentrations of serotonin, dopamine, and norepinephrine through release. Tramadol inhibits the reuptake of serotonin and norepinephrine and weakly stimulates μ-opioid receptors. Together, these substances significantly increase serotonergic activity. MDMA causes a large amount of serotonin release, and tramadol prevents it from being rapidly cleared, which can amplify the serotonin stimulus.

  2. Therapeutic effect: In therapy, no additional positive therapeutic effect of this combination is known. Tramadol may provide additional pain relief (MDMA is also mildly analgesic), but psychologically, the combination is no more beneficial than MDMA alone. MDMA's empathogenic effect remains intact as long as serotonin is available, and tramadol blocks reuptake, which MDMA itself does not prevent.   3. Danger or risks: The combination carries a very high risk of serotonin syndrome. Together, they can lead to a life-threatening accumulation of serotonin (fever, muscle tremors, convulsions). The risk of seizures is also higher because both have convulsive potential. Furthermore, tramadol, as a weak opioid, increases the risk of respiratory depression, especially in combination with other antidepressants (although MDMA is primarily a stimulant). In combination, unexpectedly severe blood pressure changes may occur, and the likelihood of feeling unwell (nausea, sweating, dizziness) may be greater.
