The use of tramadol in combination with psychedelics such as psilocybin or LSD is advised against. Tramadol is an opioid painkiller that affects the serotonergic system, and combining tramadol with psychedelics can increase the risk of serotonin syndrome increase. This syndrome is a life-threatening situation that can arise from excessive serotonin activity in the brain. It is therefore better to first complete treatment with tramadol and taper off the medication before switching to the use of psychedelics, even if it is only microdosing. With microdosing, you may use fewer psychedelics, but it can still be dangerous.

Combining it with psychedelics increases the risk of serotonin syndrome, a potentially life-threatening condition in which too much serotonin is active in the brain.

Even with microdosing, the risk of serotonin syndrome is real as long as tramadol is still active in the body. The best and safest approach is to first stop taking tramadol completely, in consultation with a doctor, and only then consider starting microdosing.
