Can Psilocybin and Tramadol be taken together?

Psilocybin + Tramadol can cause:

  1. Neurochemical interactions: Psilocybin activates 5-HT₂A, while tramadol has SNRI activity and μ-opioid agonism. Both increase serotonergic activity: psilocybin through receptor activation, tramadol through inhibition of serotonin reuptake. Like LSD, this can lead to a synergistic increase in serotonin.

  2. Therapeutic effect: Combining psilocybin with tramadol theoretically offers little additional therapeutic benefit. Psilocybin therapy focuses on psychological effects via serotonin, to which tramadol adds little other than pain relief. The quality of the psychedelic experience presumably remains the same, except that tramadol may provide some sedation. Suppressing pain with tramadol can increase comfort during the session, but it is unclear whether this positively influences therapy outcome.

  3. Danger or risks: The greatest danger is serotonin syndrome. Both substances can contribute serotonergically, and their combination can lead to excessive serotonin activity. Although no case reports are known specifically for psilocybin + tramadol, tramadol is known for SS events (serotonin syndrome) and psilocybin is related to other hallucinogens that can trigger SS. Therefore, caution is advised. Other risks are similar to those of LSD + tramadol: epilepsy (The combination can therefore increase the risk of an insult). Psilocybin generally does not cause respiratory depression, but tramadol does; in combination, this can cause mild respiratory depression, especially at high doses. Finally, there is an increased risk of sympathetic nervous system overstimulation: both tramadol and psilocybin can slightly increase heart rate and blood pressure, especially when used together. To date, psilocybin + Tramadol is strongly discouraged due to risks of serious to potentially fatal outcomes.
